Services to those with Intellectual Disabilities (PFDS/Consolidated Waivers)
This program offered under the Department of Behavioral Health/Developmental Disabilities is designed for individuals 18 years of age and older who have been diagnosed with an intellectual disability. Care is provided under Home & Community Habilitation, Companion, or Respite services.

How to Register:
1.) CALL your local county Office of Intellectual Disability
Philadelphia: 215-685-4677

2.) Schedule a meeting with the office and bring the following:
- Social Security Card
- Birth Certificate
- Proof of Address
- Medical Assistance Card (if you do not have one, you will have to apply for it)
- Consumer’s Financial Information
- Recent Psychological Evaluation: must include:
- Proof of disability which occurred before 22nd birthday
- IQ score of 70 or below
- Limitations in at least 3 of these areas: communication, social/interpersonal skills, self-care, self-direction, leisure, functional academic skills, work, health, safety, home living, use of community resources
At the meeting, the county office will help you complete paperwork for registration and apply for waiver, if applicable. You should hear back within 30 days to see if you are eligible. If you are, then you will be given a choice of a supports coordination (case management) organization. If you are not eligible for a waiver, you may be placed on a waiting list. Once you are assigned a support coordinator, they will be able to help you get the services you need.
