Home Care Program



PASSi believes that effe
ctive home health care delivery is achieved when clients and their caregivers can communicate with each other.  

To fill this critical need, PASSi delivers services in ten languages including Cantonese, Khmer, English, Korean, Mandarin, Vietnamese, Indonesian, Tagalog, Hindi, and Gujarati. Our Home Health Aides share a cultural and linguistic background with the clients they care for, further enhancing their quality of life.  

Services Offered to Clients: 

  • Regularly scheduled home visits 
  • Asian meal preparation & feeding assistanceb Rodriduez
  • Medication reminders
  • Assistance with bathing
  • Grooming & dressing
  • Light housekeeping
  • Laundry & linen washing
  • Companionship & conversation
  • Assistance with hygiene
  • Shopping for groceries or clothing
  • Respite care for family caregivers
  • Hospice Care

The home care program is available to all individuals that functionally qualify for home care services, without any regard to race, color, national origin, sex, sexual orientation, age or disability.

How Can I Obtain These Services?

Simply call us at 215-572-1234 if you or your family member needs help with daily living activities such as bathing, grooming, dressing, or preparing meals.

How Long Will The Process Take?

It usually takes 1-2 month(s) from the date of the initial call.  PASSi refers the case to the local Area Agency on Aging (AAA).  The AAA assessor evaluates the client’s health condition.  If the client qualifies, PASSi will receive a service order from the AAA to send a home health aide to the client’s home for a specified number of  hours per week.

How Much Does It Cost?

Services are available at no cost to qualified, low income individuals.

Aging Waver

This waiver program is designed for disabled individuals age 60 and older who require nursing facility level of care. 

Services include:

  • Personal Care
  • Companion Services
  • Personal Assistance Services (PAS) - bathing, dressing, cleaning, meal preparation, shopping
  • Homemaker Services
  • Respite Care
  • Home Delivered Meals
  • Specialized Medical Equipment and Supplies
  • Emergency Response Units

Family members (other than a spouse) may be hired as a paid caregiver under PAS (Personal Assistance Services).

Attendant Care Waiver

This program is designed for individuals age 18-59 who have a physical impairment lasting 12 months or more and require nursing facility level of care. 

Candidates must be mentally alert and capable of directing their own care. 

Services include:

  • Personal Assistance Services (PAS) - bathing, dressing, cleaning, meal preparation, shopping
  • Personal Emergency Response System

 

Family members (other than a spouse or parent of a minor) may be hired as a paid caregiver under PAS.

Independence Waiver

This program is designed for individuals age 18-59 and older who have a physical disability that is likely to last indefinitely combined with substantial functional limitations. 

These may include mobility and community limitations requiring nursing facility level of care.  Qualified individuals must not have a primary diagnosis of mental health, mental retardation or be ventilator dependent to qualify.

Services include:

  • Personal Assistance Services (PAS) - bathing, dressing, cleaning, meal preparation, shopping
  • Respite Care
  • Home Modifications
  • Assistive Technology
  • Personal Emergency Response System

 

Family members (other than a spouse or parent of a minor) may be hired as a paid caregiver.

Options Program

Individuals that do not meet the financial and/or functional eligibility guidelines may be approved for services under the Options Program.

HHA Class Information

PASSi is now accepting applications for our Home Health Aide classes. The classes will be taught in various languages including English, Korean, Chinese, Khmer/Cambodian, and Gujarati.  This two-week program offers 80 hours of instruction. Please call 215-572-1234 for more information.
